Due to Covid, the Illinois Department of Labor has issued a new version of the work permit.
This is an example of what the new work permit should look like. The two differences are the question at the top about regular / performer employment and at the bottom question about COVID-19 protocol. Even if you recently updated your child's work permit, please note - if it is not this exact form - it is not valid. Please check your child's work permit, and renew if needed with the correct form. If your school does not have the form, they should be able to get it from the IDOL's website, and/or the Board of Education in Chicago should issue the correct form.

We must have FOUR CURRENT photos of your child uploaded to their POP profile: - CURRENT LOOK: FRONT - a photo of their face - BODY ALTERNATIVE OR RECENT NON-PRO BODY SHOT - a full body shot - CURRENT LOOK: SIDE - CURRENT LOOK: BACK - a full body photo with hair down
The photos should be in good lighting with a neutral background, no filters, no hats, no sunglasses. Please write the date on a piece of paper and hold it up in the shot so we know for certain on what date the photo was taken.
The photos must accurately reflect your child's skin tone. You must upload the photos from a computer, not your phone. If you have a photo in a spot already - delete it first, then refresh the page, and then upload the new photo into the selected spot. If you have tried this multiple times and are not able to replace the photos, then send an email to [email protected] with the subject line: UPDATED PHOTOS FOR MINOR'S PORTAL. It is vital that you include the name of your child and the email attached to their profile for us to be able to help you.
