State of Arizona Minor's Registration

Arizona Child Labor Laws exempt child (persons under the age of 18) performers in motion pictures, theatrical, radio, or television productions, so long as the production company provides the Department of Labor of the Arizona Industrial Commission with the name, and address of the person, the length, location, and the hours of employment, and any other information required by the department before the production begins (AZ Statute 23-235). **All minors must either have 1 document from Column A or two documents - one from Column B and one from Column C from the I-9 in order to complete payroll paperwork.

Once the parent and/or guardian have signed the minor affidavit (for all minors under 18) and saved as a PDF and you have obtained the minor's ID, please save them as a jpeg.  After you complete the short form below, you will be taken to the background application page via our EP Casting Portal website.
